As an Associate Medical Director at CenterWell Home Health, you will play a key leadership role in advancing high-quality, patient-centered care in the home setting. CenterWell Home Health provides personalized, at-home healthcare services that enhance patient autonomy and quality of life through skilled nursing and therapeutic treatments, and your clinical expertise will help ensure that every patient receives safe, effective, and compassionate care. Working within the Healthcare & Medical Services department, you will collaborate closely with physicians, nurses, therapists, and other members of the interdisciplinary team to guide clinical decision-making, coordinate complex care plans, and promote evidence-based practices across the service line. You will provide medical oversight and support for home health services, helping to shape and implement clinical protocols, pathways, and standards that meet or exceed regulatory and accreditation requirements. With a strong focus on patient safety, quality outcomes, and appropriate utilization of services, you will review clinical performance data, participate in case reviews, and help resolve complex clinical issues that arise in the field. You will serve as a clinical resource and mentor for field clinicians, providing guidance on best practices, addressing questions related to diagnosis and treatment, and supporting continuous professional development. Aligned with CenterWell’s culture of compassion, integrity, and excellence, you will champion initiatives that improve patient satisfaction, reduce avoidable readmissions, and strengthen coordination with community physicians and hospitals. You will contribute to performance improvement projects, incident reviews, and corrective action plans, ensuring that lessons learned translate into safer and more reliable care for patients at home. In partnership with operations, quality, and compliance leaders, you will help maintain adherence to federal and state regulations, payer requirements, and internal policies specific to home healthcare. The Associate Medical Director will also participate in the development and delivery of education and training programs for clinicians, including orientation, ongoing competencies, and targeted instruction on emerging clinical practices, regulatory changes, and documentation standards. You will be expected to use data and analytics to identify trends, support decision-making, and drive improvements in clinical outcomes and efficiency. In this role, you will regularly engage in interdisciplinary case conferences, collaborate with referring and primary care providers, and represent CenterWell Home Health in internal and external clinical forums. This full-time role typically offers a hybrid work environment, combining remote work for administrative, review, and coordination tasks with occasional field or on-site presence for meetings, case conferences, and collaboration with clinical teams.
