House Manager Position Summary: This position will oversee the residential care staff and ensures smooth daily operations of all communities associated with the organization. The role offers services to residents as indicated in each resident’s individual service plan within multiple communities associated with Sage Oak Assisted Living and Memory Care . In addition to residential care and operations of the home, this position provides leadership and a point of communication for all current and new employees, residents, and family members. The ideal personnel will be able to communicate and demonstrate positive company culture. Personnel will contribute their talents and energies to further improve the quality and environment of those whom we care for. This position requires teamwork and critical-thinking skills with a problem-solving attitude to contribute to the daily clinical and operations of multiple communities. It also requires team management and leadership skills paired with outstanding communication skills. This position will serve as a role model for others by displaying a responsible, cooperative, and positive attitude with a willingness to help out when scheduled. It will provide support to residents and hands‑on assistance to fellow staff, assist in the development and revision of service plans, observe and report any issues or concerns surrounding residents and their care, and perform certain food services, activities, environmental duties and assist with medication management. Position Supervisor: Sage Oak Management and/or Administration Responsibilities Maintain the comfort, privacy, and dignity of residents in the delivery of services and interact with residents in a manner that displays warmth and promotes a caring environment. Demonstrate a positive, cheerful attitude and treat all residents, families, supervisors, and co‑workers with dignity and respect. Communication and interactions are effective and tactful. Demonstrate and facilitate the company philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents. Administer quality care to meet the needs of each resident. Supervise or assist residents with performance of activities of daily living, simple treatments, and emotional health or problem behavior issues as indicated by the individual service plan. Redirect or de‑escalate residents when emotional or problem behavior issues occur. Perform other services as indicated in individualized service plans (laundry, meals, pet care). Prioritize resident care according to need and acuity. Maintain and ensure the community is organized and operates smoothly to ensure daily operations are not impeded. Assist with medication administration, handling, supervising, ordering, and storage, including: Provide medication assistance for residents in accordance with state regulation and company policy including pouring, preparing, assisting with administration, and documentation. Ensure proper handling and/or administration for residents. Conduct accurate medication counts for narcotics and store medications in the medication cart. Maintain compliance with company policies and procedures related to controlled substances and documentation. Complete medication training and/or delegation as necessary. Manage special medication (injectables). Demonstrate successful passing of medication to residents without error. Create medication administration records on a monthly basis or as needed for residents and audit them for accuracy against physician orders. Assist with medication ordering in a timely fashion to comply with the resident’s service plan. Ensure adherence to medication intake and documentation in the medication profile record (MPR) for each resident. Audit, file, and maintain all pharmacy and physical orders for residents on a monthly basis or as needed. Adhere to and assist in the development and revision of individual service plans by communicating with the Resident Care Coordinator and/or Management for resident needs and preferences. Observe and report residents’ issues or problems to management in a timely fashion. Continually stay in tune to each resident’s clinical needs and acuity. Identify and follow appropriate meals and/or diets for residents. Alert Management and/or Resident Care Coordination if unsure to complete a task as indicated by the service plan. Assist in maintaining the integrity of a resident’s skin by ensuring completion of weekly skin assessments and completing documentation while on shift. Identify signs and symptoms of compromised skin integrity. Observe and report skin issues or problems to management in a timely fashion. Ensure completion of appropriate documentation about issues observed by direct staff. Adhere to and assist in the development and revision of resident preferences for each resident as needed by collaborating with resident/family and/or Resident Care Coordinator. Perform and document proper measurements of vitals daily or as needed according to the service plan or otherwise directed. Notify the facility supervisor of any needed medical supplies to be purchased. Perform multiple proper transfer techniques as indicated by the individual service plan for residents, including gait belt, pivot, Hoyer lift, and/or sit-to-stand. Observe and report if direct staff need additional training on proper transfer techniques, and demonstrate proper transfer techniques for direct staff. Communicate professionally and timely with fellow teammates, including Executive Director, Resident Care Coordinator, outside services or vendors (Home Health, Hospice, pharmacy, etc.), and family members. Effectively communicate with all residents, families, staff, visitors, and management in a positive, helpful, and productive manner. Perform food service duties, including cooking meals, serving meals, delivering meal trays to rooms, bussing tables, cleaning dining rooms after meals, setting tables, and adhering to the daily and weekly menu as needed. Compose or create daily and weekly menus to be provided to direct staff. Ensure direct staff communicates with the facility supervisor before substitutions are made on the daily and weekly menus. Ensure the daily, weekly, or monthly menu is posted and filed accordingly. Be present at mealtimes to prevent choking incidents for residents. Communicate and document appropriately with Resident Care Coordinator and/or Management about meals refused or not consumed. Ensure compliance with the plating method when preparing and presenting meals to residents. Collaborate and create a shopping list for each community on a weekly basis and order or pick up orders as needed. Perform activities duties as assigned by management and/or administration, including leading group activities, engaging residents in activities, and accompanying residents to facility-sponsored activities outside the facility. Assist with development and revision of daily activities as needed. Offer and encourage activities based on the activity calendar, including offering group or individual activities. Ensure pictures are taken of activities and/or events planned and send to management as needed. Post activity calendars throughout the community and file them accordingly. Record incidents, errors, and accidents and report resident concerns to Management and/or Resident Care Coordinator immediately regardless of whether medical treatment is required. Ensure adherence to incident reporting policies and procedures. Collaborate with fellow staff to observe changes in resident status, needs, or preferences. Ensure completion of incident reports and appropriate documentation upon discovery of incidents or accidents by direct staff or self. Track, manage, and report all incidents with residents, staff, or families on a daily basis to management and/or administration as needed. Perform all housekeeping duties as indicated on daily assignment sheets, assisting with environmental services while on shift, including sweeping, mopping, washing and drying resident laundry, deep cleaning common areas and resident rooms, and cleaning restrooms. Perform additional environmental services as directed by Management. Create, revise, and implement daily assignment sheets for direct staff. Read the communication binder at the beginning of each shift and demonstrate knowledge of the communication provided. Sign off on communications from Administration and ensure communications are printed and placed in the binder for direct staff. Facilitate all need-to-know information to direct staff through a communication book, text, email, or phone call. Ensure verbal communication and written reports at the beginning and end of shift change happen for direct staff. Gather report the Sage Oak Way. Document all care provided in the Observation Book with clarity, accuracy, and conciseness. Attend educational and/or mandatory facility meetings monthly to ensure fulfillment of state or provincial regulations. Create, lead, and facilitate monthly in-services for all direct staff within the community. Create and ensure completion of comprehensive assessments for all direct staff covering continuation of educational credits. Maintain the ability to lift fifty (50) pounds or more, stand or walk for more than an eight (8) hour workday, and be able to push, pull, bend, twist, and reach multiple times throughout a shift. Track, manage, and report attendance and tardiness issues to management and/or administration on a weekly basis. Adhere to incontinence, toileting, repositioning, and visual checks of residents during the duration of the scheduled shift: provide incontinence checks every two (2) hours or as indicated by the service plan, provide toileting assistance every two (2) hours or as indicated, and monitor residents’ safety by visually checking every forty-five (45) minutes or as indicated. Assist with training of all new hires and/or employees as directed, completing sixteen (16) hours of on-the-job supervised training covering various areas such as ADLs, health conditions, prevention of accidents and injuries, first-aid procedures, managing disruptive behaviors, fall prevention, and emergency evacuation procedures. Ensure new direct staff understand and demonstrate knowledge about time sheets, narc count and documentation, MARS, communications, shower logs, vital signs, assistance with medication, accident and incident reporting, sign-in and sign-out sheets, skin assessments, grievance reporting, MPR, service plan, and resident charts. Ensure completion of additional on-the-job and/or state training in areas such as reporting abuse, neglect, or exploitation, HIPAA, infection control, resident rights, emergency and evacuation procedures, cooking proficiency, ADLs for Alzheimer’s and/or memory loss residents, managing dysfunctional behaviors, and identifying and alleviating safety risks for Alzheimer’s and/or memory loss residents. Observe and report any issues with management and/or administration during the training period and ensure documentation is complete and administration notified. Assist with placement and scheduling of new direct staff as needed or directed, and scheduling and facilitating training and re-training of employees with management and/or administration. Ensure tracking of all inventory or supplies purchased. Conduct monthly audits of all binders to ensure compliance with local, state, or federal guidelines and continue completion for all binders assigned. Audit the state survey binder and all resident charts for proper filing of documentation and missing information as needed. Track, manage, and report any maintenance issues within or outside the home on a weekly basis or as needed. Actively participate in scheduling of communities, ensuring all shifts are covered and acting as a last resort for coverage if none found. Demonstrate willingness to assist and cover shifts as needed at multiple locations or communities as directed. Ensure completion of or fulfillment of individual community schedules to ensure coverage on a weekly basis and communicate scheduling needs and/or changes to the scheduling manager to ensure continuity. Assist with community tours as needed or expressed by management and/or administration.
