Preferred Qualifications Preferred: Bachelors of Sciences in Nursing Will Consider: Licensed Practical Nurse with 2+ years of related experience Valid Illinois driver’s license, auto insurance, and reliable transportation What You’ll Do Provide Care: Administer pediatric and adult immunizations Assess Needs: Conduct health, developmental, and psychosocial assessments Plan & Follow Up: Develop care plans and ensure continuity of services Educate: Deliver culturally appropriate health information to individuals and groups Collaborate: Work with the nursing team to coordinate client care Maintain Records: Keep accurate documentation of all client interactions Community Outreach: Participate in outreach activities and public health initiatives. Programs include Immunizations, TB, Lead, WIC, and STI prevention. Knowledge of state and federal public health regulations required. Why Join Us?
